(Please mail only copies and provide as much information as possible. If a part of information is not available or does not apply, send us what you do have.)

	debtor contact information (complete name, address, phone numbers)
	contact information for any other responsible parties (spouse, management company, etc.)
	written agreement between you and/or your company and the Debtor
	debtor credit application
	initial and all subsequent statements / invoices
	returned check
	any collection letters or other correspondence pertaining to the debt that you have already sent
	balance owed to you or your company at the time you send us the file as well as an explanation of the balance (added late fees, interest, etc.)
	any and all other information you think might help us in the collection of the money that is owed to you

Please provide as much information as possible regarding the specifics of the debt. Our success in collecting your debt may hinge on the detail of the information you provide. This is not to say we cannot collect your debt if you have limited information. It just means that the "details" greatly aid in the debt collection process.
